I had been wondering if it was related to your hospital inflicted back issues of a few years ago.
How does your saddle differ from the one on your road bike (assuming that is comfortable).
I departed from my pure Italian thoroughbredness to put a specialised taupe road saddled on it - mainly because of the sit bone measurement and width choices.
I then picked up the (no longer made) TT version of the same in the same width (seems to have shorter nose). I can't recall any additional discomfort, even with a 2.75 hour ride on it (and I hardly do any miles on the TT as it isn't suitable for riding except without a car assisted ride).
I get less shoulder neck issues on the TT than on the road bike (and certainly less than when driving...).
There may well be some good exercises (I did get some off the physio for shoulder neck at one time).
